Method of application Applicant should submit an application through the following channels:

1.

Written application: Submit the application in person at the DICJ office (Download Forms)

2.

Electronic application: Apply at Sites of Self-Service Application
Imperative documents

Applicant should submit the followings despite the applying channel:

Valid identification documents

2.

A recent ID photo

Relative services DICJ will notify the applicant to sign the notification letter within five working days after the application is submitted. (Performance Pledge )

Servicing target The applicant must be the excluded person’s spouse, parent, son/daughter or brother/sister.
Condition of application: The person to be excluded must sign on the application form to confirm the exclusion.
Method of application Written application: Submit the application in person at the DICJ office (Download Forms)
